Forms required prior to initial consultation:
Customer Agreement - Please read, fill out, initial each page, and sign the last page.
New Client Form - This is required for each pet. Again, please read, fill out, initial each page, and sign the last page. Please note that you will also need to include proof that your pet has received all of its vaccinations.
Also, if you wish you pay by credit/debit card:
Credit/Debit Card Authorization - This is only required by customers choosing to pay by credit/debit card. Please note that this form must be signed in the presence of a Busy Bark LLC member/owner or employee. You may also be asked to present your state-issued driver's license (or passport) and your physical credit/debit card at that time for verification.
